O jogo mantém a elenco original de Real Bout, com a adição de Tung Fu Rue, Cheng Sinzan, Laurence Blood e Wolfgang Krauser do Fatal Fury Special, com o Krauser servindo como novo chefe final. Geese Howard, que foi morto no desfecho da versão original de "Real Bout", aparece neste jogo como um chefe final oculto em uma especial "Nightmare Match" e como personagem jogável desbloqueável nas versões domésticas. O jogo também apresenta versões extras ocultas de Tung Fu Rue, Billy Kane, Blue Mary e Andy Bogard, totalizando 23 (ou 24 se contarmos Geese) personagens.
